Step 1) Screening Questionnaire
Applications will be accepted throughout the year, please be sure to state what semester you intend to start the program. Please email your questionnaire to Captain Timmerman at bttimmerman@usf.edu and Kevin Rosario at kevinrosario@usf.edu. Remember to rename your screening questionnaire following this format: Last Name, First Name Screening Questionnaire.
Step 2) Email Screening Questionnaire
Please Rename your Screening Questionnaire following this format: Last Name, First Name Screening Questionnaire. Email your completed screening questionnaire to Captain Timmerman and Kevin Rosario. An officer will respond to your email with next steps and answer any other questions you may have about the program.
